What this is about
You take damage, your screen turns red – and then? This guide sorts out the medical system: what health is, how an injury differs from it, what medication does, and where the line runs between “treat yourself” and “go to a bed”. The numbers come from the game data,q1 the descriptions from the game’s hint texts.q2
Medical basics
Four things run separately in the game:
- Health – one overall value plus a separate value per body part.q1
- Injury – a level per body part: minor, moderate, severe.q2
- Symptoms – what you notice: bleeding, being stunned, restricted movement, hypothermia and hyperthermia, radiation sickness, hunger and thirst.q1q2
- Blood drug level (BDL) – how much agent is in you. It rises with every dose.q2

Read your health status
The game shows your status in two places: as a HUD warning with body part and severity, and as a full list in your personal menu – which, according to the game text, also lists active medication with its remaining duration.q2
The warning names three levels: minor, moderate and severe injury. For each level the game text states which treatment tier a medical bed needs.q2 More in understanding injuries.
Treating
Two ways are open in the field:
- A medpen – an injector for exactly one dose, for you or for someone else.q1
- The ParaMed – a medical device that doses over a short distance and shows the target’s status while doing so.q1q2
Both relieve symptoms and improve the condition. The injury itself remains: according to the game text it is only treated fully in a medical bed of the matching tier.q2
Stabilise
“Stabilising” means in practice: keeping symptoms under control far enough that the person can be moved, without driving the BDL into the critical range. The game text explicitly describes the device as a means to stabilise patients with possibly life-threatening injuries.q2
Incapacitated
If someone goes down, a timer to death starts. During that time the controls offer two functions: request rescue (while incapacitated) and regen (while incapacitated) – both have to be held.q3 The game text advises asking your party, the chat or a rescue beacon for a revive before the timer runs out.q2
